Continuity of Care

Continuity of care is important in all walks of life but particularly so in healthcare.
The weight of evidence links Continuity of Care to improvements in mortality, morbidity, and satisfaction with care. There is a consistent association with reduced use of urgent care services. The advantages outweigh potential disadvantages such as the risk of accepting a patients wishes or seeing a patient more often with challenging problems.

The Health Foundation Programme has looked at real world interventions to increase continuity of care across five sites in England covering half a million patients. The results are emerging over 2020 and 2021. So far an increase in continuity of care has been shown for both “microteams” and individual doctors. Despite the pressures of the NHS and increased part time working both access to care and continuity of care can be achieved.
